Some users were unable to submit a form which had CSV attachments enabled between 09.37 and 11.57 on Friday 24 January.
We think the problem was confined to one form, and that 6 users were unable to submit this form. These users would have seen an error message when attempting to submit the form.

We believe this incident has now been resolved, and that users are now able to submit the form in the normal way. We’ll notify the department whose form was affected.
